Premium cataract surgery is for patients who elect to have cataract surgery to improve their vision and who also wish to decrease or eliminate their need for glasses. This may involve astigmatism and/or presbyopia correction.

Most patients who are candidates for cataract surgery and lens replacement surgery are candidates for a premium cataract surgery using a premium lens implant.

We were the first facility in Central Pennsylvania and one of the first in the nation to offer this revolutionary technology to our patients.

With premium cataract surgery, many of the steps that are usually performed manually are instead performed with the precision of a laser.

With this bladeless procedure, computer-generated images of your eye allow your surgeon to customize the cataract procedure to your unique eye offering unparalleled consistency and safety to the surgery.

Surgery works by replacing the natural lens of the eye with a premium lens.

A premium lens implant is a lens that is designed to help decrease or eliminate the need for glasses or contact lenses after cataract surgery.

There are many types of premium lenses, but they all generally fall into three categories:

  • Toric (astigmatism) correcting lens implants
  • Multifocal lenses with or without astigmatism correcting lens implants
  • Crystalens with or without astigmatism correction

A toric intraocular lens is a type of lens implant that offers patients who have astigmatism the ability to see more clearly without glasses than they otherwise would if they chose a standard monofocal lens. During your cataract surgery the amount of astigmatism on your cornea is measured. This measurement is used to order and customize the toric lens implant so that it will correct the amount of astigmatism specific to your eye. After the cataract is removed during surgery, the toric implant is placed and aligned so that it corrects your astigmatism providing great uncorrected distance vision.

A multifocal lens implant also known as trifocal implants or extended depth of focus lens implants use

Crystalens is a single-focus lens which we can implant into your eye after we have removed your cataract during cataract surgery. This artificial lens works naturally with your eyes’ muscles to give you the quality of vision you enjoyed when you were younger. The lens has “hinges” which allow the lens to move, or accommodate, to focus on objects near, far and all distances in-between seamlessly.

Unlike old-style fixed focal lenses, which didn’t move, Crystalens can move, thus focusing as your eye’s natural lens does. Your doctor will take precise measurements to ensure the correct power selection.

Limbal relaxing incisions

As a variant in premium cataract surgery without the LenSx laser, if your eye has pre-existing astigmatism, your surgeon may elect to make micro-incisions in the cornea to reduce your astigmatism. These are called limbal relaxing incisions.

The RxSight™ Light Adjustable Lens is the first and only intraocular lens (IOL) that can be customized after cataract surgery. Memorial Eye Institute is proud to be one of the only practices in Harrisburg and Central Pennsylvania to provide this cutting edge technology to our cataract surgery patients.

The Light Adjustable Lens is made of a special photosensitive material that can be adjusted in response to ultraviolet (UV) light. This optimization is done by your eye doctor in the weeks following lens implantation through a series of non-invasive light treatments that take only a few minutes each. You will have the unique ability to adjust and preview your vision until it meets your personal desires and lifestyle requirements. The Light Adjustable Lens delivers superior vision outcomes that non-adjustable IOLs cannot match.

Advantages

Premium cataract surgery can enable many patients to reduce or eliminate their need for glasses.

Disadvantages

Premium cataract surgery is not completely covered by insurance, and there are additional out-of-pocket expenses for the procedure.

The Crystalens offers significant advantages over other kinds of lens implants and corrective lenses.

  • A full range of vision: Standard (single vision) lens implants don’t have the ability to provide a full range of vision. Most people who have single vision lens implants MUST wear glasses for middle and near vision.
  • Sharper vision: The Crystalens has the unique ability to focus on objects at varying distances using the eye’s natural muscle. This means the Crystalens can provide sharper vision, without corrective lenses, throughout a full range of vision from near to far and everything in between.
  • More convenience: With Crystalens, the quality of vision compared to wearing bifocals and/or trifocals is significantly improved. You’ll have a full range of vision, without having to tilt your head to find that portion of your glasses that allows you to see clearly. You simply look at something and the Crystalens, mimicking the natural focusing ability of your eye, will automatically focus your eye at near, arm’s length, or distance.
  • Incredible results: The Crystalens FDA one-year clinical study results indicate that 92% of the people enrolled in the study could see 20/25 or better at a distance, 96% could see 20/20 at arm’s length, and 73% could see 20/25 at near without glasses or contact lenses. What is more exciting is that 98% of these people could pass their drivers test, 100% could see their computer or put on their makeup, and 98% could read a magazine, all without glasses or contact lenses. Also, your ability to see at approximately arm’s length (middle vision) is greatly enhanced with the Crystalens.
The risks with premium cataract surgery are very low.

However, like any surgery, cataract surgery does carry some risks. Here are some of those risks:

  • Eye infection.
  • Bleeding in the eye
  • Ongoing swelling of the front of the eye or inside of the eye.
  • Swelling of the retina (the nerve layer at the back of your eye).
  • Detached retina (when the retina lifts up from the back of the eye).
  • Damage to other parts of your eye.
  • Pain that does not get better with over-the-counter medicine.
  • Blurred vision.
  • Seeing halos, glare, and dark shadows.
  • Vision loss.
  • The IOL implant may become dislocated, moving out of position.

Cataract surgery will not restore vision lost from other eye conditions such as macular degeneration, glaucoma, or diabetic retinopathy.
Your ophthalmologist will talk with you about the risks and benefits of cataract surgery.

Cataract surgery alternatives

For patients with decreased vision secondary to cataracts, cataract surgery, while elective, is the only option to improve visual function. Patients who do not desire premium cataract surgery can certainly choose standard cataract surgery.

Correcting your prescription

An alternative treatment for correcting presbyopia is monovision contact lenses.

Monovision is the technique of fully correcting the refractive error of one eye and intentionally making the other eye mildly nearsighted.

In this scenario, the fully corrected eye sees distant objects clearly (but cannot see very well up close without glasses), and the mildly nearsighted eye sees very well up close without glasses (but not so clearly far away).

Monovision may sound odd the first time you hear about it. Still, this technique has been used very successfully with contact lenses for many years. It is now being used frequently with cataract surgery to decrease a person’s dependency on reading glasses and computer glasses after surgery.

Any combination of premium contact lenses can be used for monovision cataract surgery.

You will need to arrive at the Memorial Eye Institute’s surgery center entrance about an hour before your procedure.

Once the team has checked you in, they will offer you a sedative to help you relax. Our nurses will dilate the pupil in your operative eye and prepare you for surgery.

Prior to going to the operating room, the nurse will escort you to the laser room where the LenSx® laser will perform a precise and customized refractive laser treatment. This treatment will create incisions into the cornea and onto the cataract. Following the laser treatment, the nurse will transport you to the operating room.

Once in the operating room, the nurse will clean the area around your eyes and apply a sterile drape. We will use eye drops to numb your eyes and, if necessary, an anesthesiologist will administer intravenous sedation and monitor your vital signs during the procedure. We will place an eyelid holder between your eyelids to keep you from blinking during the surgery.

The doctor will then open the microscopic incisions previously made by the LenSx® laser on the cornea and using a tiny ultrasonic probe will fragment and remove the cataract.

Once the doctor has removed all of the cataract particles from the eye, they will insert a folded intraocular lens (IOL) through the micro-incision. Here the lens unfolds into the proper position behind the pupil. We can also correct pre-existing astigmatism at this time with a toric (astigmatism) intraocular lens. Alternatively, we can implant a multifocal or accommodative intraocular lens which will improve your visual function for a full range of distances without the need for glasses.

After the procedure, you will go to the recovery room where the nurses will go over your post-operative care before you are discharged. Because you may have received some sedation during surgery, you will need transportation home. We encourage you to relax for the rest of the day. Everyone heals somewhat differently, but many patients report improvement in their vision almost immediately after the procedure. Most patients return to their normal activities within a day or two.
